Teacher Registration Form

This is for SAM/SAA members to register their students for SAM Graduation. We are using online forms this year to help avoid mistakes in student names & other information for trophies and programs. 

Guidelines for completing Teacher Registration Form:
1) After you complete the Teacher Registration page, you will need to complete the Student Registration for each student participating.
2) If you have more that ten students, please complete the Teacher Registration several times until you have listed all students.  However, you don’t need to enter all data on the second and subsequent forms, but just list your name and SAA number.
3) Please check information submitted carefully.  This information will be used for programs & trophies.
